Health & Safety Checklist
Please confirm that you do not have any of the following:
- I do not have a pacemaker or any implanted electronic device.
- I am not pregnant.
- I do not have active cancer or a history of skin cancer.
- I do not have epilepsy or a history of seizures.
- I do not have a known sensitivity to light or photosensitive conditions.
- I do not have any open wounds or infections in the treatment area.
- I am not currently experiencing a fever or acute illness.
- I have not had recent surgery in the treatment area.
- I am not taking medications that cause photosensitivity.
If you are uncertain about any of the above, we strongly advise that you consult with your GP before proceeding with treatment.
Waiver of Liability
- I understand that PEMF and Red Light Therapy are non-invasive wellness treatments. I confirm that I have disclosed all relevant health information to the best of my knowledge. I acknowledge that I am voluntarily choosing to undergo this treatment and accept full responsibility for any outcomes.
- I understand that this is not a medical treatment and is not intended to diagnose, treat, or cure any medical condition.
- I hereby release the provider and facility from any liability arising from the use of PEMF and Red Light Therapy.
- If I am unsure about any of the information I have received regarding this wellness treatment, I agree to consult explicitly with my GP to ensure it is safe and appropriate for my wellbeing.
Terms and Conditions
Booking & Cancellation:
Sessions must be booked and paid for in advance.
Appointments cancelled with more than 24 hours’ notice: Free of charge.
Appointments cancelled with less than 24 hours’ notice: £20 fee applies.
Appointments missed with no contact or notification of cancellation: Full cost of the appointment will be reserved.
There may be times when Calm Together has to cancel your session due to unforeseen circumstances. In this case you will be notified by email and given a full refund.
Payment:
Payment is due at the time of booking unless otherwise agreed.
Privacy
All personal and health information provided will be kept confidential and stored securely using GDPR compliance. You consent to us holding your personal information.
We do not share your data with third parties without your consent.
Facility Use
Please arrive on time for your session.
Mobile phones should be silenced during treatment.
Respect the space and others using the facility.
Clients are responsible for maintaining the cleanliness of the room. Please leave the room as you found it, removing any rubbish and personal items.
Due to the technological nature of the treatment, no food or drinks are to be consumed in the therapy room.
Clean towels are provided for hygiene purposes. Each client must use a fresh towel on the bed for every treatment and place it in the designated laundry basket at the end of the session.
Right to Refuse Service
We reserve the right to refuse treatment if safety / conduct issues arise.
GP Consultation Statement
If I am uncertain about my health status or suitability for this treatment, I agree to consult with my GP before participating.
